We are looking forward to getting started, and we encourage everyone to take full advantage of our Wellness Program. One’s health impacts every aspect of life – family, friends, work, and play. The wellness program will help you make beneficial choices for a healthier life. On this page, you will find the various resources available to you as well as the key steps to participate in the wellness program.

This is a voluntary wellness program available to all employees. If you choose to participate in the wellness program you may be asked to complete a voluntary health risk assessment or "HRA" that asks a series of questions about your health-related activities and behaviors and whether you have or had certain medical conditions (e.g., cancer, diabetes, or heart disease). You may also be asked to complete a biometric screening, which will include a blood test for cholesterol and glucose. It may also include blood pressure, height and weight, BMI, and waist circumference. You are not required to complete the HRA or to participate in the blood test or other medical examinations. However, employees who choose to participate in the wellness program will receive an incentive for participating. The information from your HRA and the results from your biometric screening will be used to provide you with information to help you understand your current health and potential risks, and may also be used to offer you services through the wellness program, such as coaching, lifestyle management and fitness challenges. You also are encouraged to share your results or concerns with your own doctor.

Step 1 Participate in a Biometric Screening

Your employer is helping you become a “better you” by offering biometric screenings to help improve your understanding of your health and well-being. We do this by providing new insights to your health risks through laboratory results that include your cholesterol levels and your blood sugar. This is a non-fasting screening so you can go about your day as usual.

Please remember that all information obtained in your screening is personal and confidential, as protected by federal law. Your employer will not have access to your individual results. We encourage you to share your screening results with your physician during your next visit.

Step 2 Complete a Health Risk Assessment

After your screening, you will complete the online Health Risk Assessment (HRA). The HRA considers aspects of your health, illnesses you’ve had and any other lifestyle issues. When you have this knowledge, you have more control and can start making simple changes to improve your health. When you are ready, click on the Log into your HRA link below. Make sure to have your screening results with you!

Your username is your Banner ID.

Use your current password if you have logged in before. If this is your first login, your password is set to Wellness@WSU, which you will be required to change on first login.

After logging in, click the 2018/19 WSU HRA button to complete your HRA.

Step 3 Share Your Results


Do yourself a favor and share your results with a loved one and take a look at your risks together, then tell them one behavior you will change today. It’s great to have someone cheering you on!

As a participant in the Wellness Program, you will have the opportunity to participate in wellness activities that encourage a healthy lifestyle. Be on the lookout for announcements that detail these upcoming activities! Remember, this is a free and completely confidential program. No individual health information is shared with the company.

Step 4 Unlimited Telephonic Wellness Coaching

Everyone who completed the biometric screening and took the HRA is eligible to receive wellness coaching. These sessions are done on a one-on-one basis over the telephone. The coaches identify motivators, obstacles and strategies that will lead to healthy living. Step 5 Visit the Life Advisor Wellness Portal

Our online portal gives you a personal and private page to track your biometrics, review your HRA, chat with a health coach, and more!

Please use the login information you used when you completed your HRA.

If you provided us an active email address, and you forgot your password, you can use the Forgot Password? Link available on the login page.

Step 6 Visit the Life Advisor EAP Website

The Employee Assistance Program (EAP) benefit provides support and resources for employees, their significant others and dependent children. Consultants are available 24/7/365 via phone to connect you with a counselor close to work or home, phone counseling, referrals or work-life materials. Call 1-800-448-8326 to access the Life Advisor EAP services. The services are completely confidential and at no cost to you.

We are required by law to maintain the privacy and security of your personally identifiable health information. Although the wellness program and your employer may use aggregate information it collects to design a program based on identified health risks in the workplace, Ulliance will never disclose any of your personal information either publicly or to the employer, except as necessary to respond to a request from you for a reasonable accommodation needed to participate in the wellness program, or as expressly permitted by law. Medical information that personally identifies you that is provided in connection with the wellness program will not be provided to your supervisors or managers and may never be used to make decisions regarding your employment.
