To enroll in your Employee Wellness Program you may have to complete a Biometric Health Screening and/or a Health Risk Assessment (HRA). Your Biometric Health Screening is conducted by University Pharmacy, and includes taking your height, weight, waist circumference, blood pressure, total cholesterol, LDL, HDL, triglycerides and glucose (fasting or non-fasting will determine which metrics are tested). The on-line Health Risk Assessment questionnaire includes questions about your health measurements, health habits and overall lifestyle. Following your completion of the HRA you will be able to immediately access a confidential report of your overall health and health risks.
All of your health information, including your Biometric Health Screening results and Health Risk Assessment (HRA) data, is confidential. It will never get back to Human Resources, your insurance or your supervisor. Your Human Resources department receives aggregate data, meaning data that summarizes the company as a whole, not with individual results. Your Human Resources department will not see your screening results, risk levels or overall wellness score. Ulliance provides the Human Resources department with demographic information such as first and last name which is used to determine if you qualify for the incentive.
Ulliance serves as your wellness vendor administrator and may provide your individual health results to our Wellness Coaches, Smoking Cessation Provider and other designated Wellness Service Partners affiliated with your Employee Wellness Program.
Your health data is held in compliance with HIPAA, the laws governing personal health information. If your employment with the company ends, you are no longer able to participate in the wellness program.
